Medicaid Service Coordination

Medicaid Service Coordination (MSC) is a service offered to individuals with developmental disabilities, which assists them in gaining access to the necessary support and aid needed to live productive lives.

Arc of Yates provides MSC to individuals with developmental disabilities by qualified staff that uses a person-centered planning approach. Person-centered planning focuses on the needs and desires of the individual receiving services while striving for the highest quality of life.

Medicaid Service Coordinators assist individuals to achieve goals and desires. Along with the individual receiving services and their circle of natural support, the Medicaid Service Coordinator develops an Individualized Service Plan (ISP) which outlines what goals the individual hopes to accomplish, the strategies to achieve these goals, and what support they will receive along the way. Plans are evaluated and updated as the individual progresses and to ensure all support and services are meeting the individual’s needs.

Medicaid Service Coordinators assist individuals with services such as future planning, residential planning, recreation, residential habilitation, day habilitation, reimbursement, adaptive equipment, and environmental modifications.

To be eligible for Medicaid Service Coordination, you must have a documented diagnosis of a developmental disability demonstrating the disability prior to the age of 22, be enrolled in Medicaid, and demonstrate a need for ongoing and comprehensive service coordination.
